Health and Immunization Requirements

Kindergarten and 1st Grade (under age 7)

DTP, DTaP, DT, Td
(any combination) 		4 doses with one after age 4
Polio (OPV, IPV)
(any combination) 		3 doses with one after age 4
Measles 2 doses after age 1 and 28 days apart
Mumps 1 dose after age 1
Rubella 1 dose after age 1
Hepatitis B
(HepB)		 3 doses
Varicella 1 dose after age 1 or history of disease

Notes:

  1. Personal exemptions expire after 2 years.
  2. Four day grace period allowed on any age minimum or interval.
