Health and Immunization Requirements

Kindergarten - 12th Grade (age 7 and above)

DTP, DTaP, DT, Td
(any combination) 		3 doses with one after age 4 - the last dose must be less than 10 years old
Polio (OPV, IPV)
(any combination) 		3 doses with one after age 4
Measles 2 doses after age 1 and 28 days apart
Mumps 1 dose after age 1
Rubella 1 dose after age 1
Hepatitis B
(HepB)		 3 doses
Varicella 1 dose after age 1 or history of disease

Notes:

  1. Personal exemptions expire after 2 years.
  2. Four day grace period allowed on any age minimum or interval.
  3. Age 4 boosters given before 08-01-2004 have a grace period allowing the booster to be given in the month preceding the birth month.
  4. Varicella started at age 13 requires 2 doses
  5. Polio vaccine is not required for persons 18 years of age or older.
